Reason feels that an informed culture is one where the management is fully informed on the system that they are managing. Singapore Flyer Stopped 3 Times Before

Straits Times reported that the Singapore Flyer (a larger version of the London Eye) which stopped about 3 weeks ago, had similar incidents before. The previous incidents were relatively minor as compared to the Dec 23 08 incident, where 173 passengers were stuck in the wheel for more than 6 hours.

Humanising safety - caring culture

Recently attended a workshop by Optimus Safety Management which focused on the importance of a caring culture and ensuring that safety is not de-humanised. It is so easy to see safety as just another form or just a number, it frequently takes a third party to help us refocus on why safety is important.

I've attended a similar workshop by JMJ Associates a few years back and they have a similar message of not degenerating safety into numbers and treating each injured as a case. However, it is amazing how easily these important messages get lost when work pressure mounts and caring for another person becomes a luxury...
